Any sublicense, assignment, or transfer to third parties would also have no restrictions on the use of your Personal Data. The Personal Data shared may include information regarding your name, date of birth, demographic information, medical history, physical and psychological conditions, diagnoses, treatment recommendations, images, studies, labs, and any other information you share with your Provider. The information shared under the License Agreement may also include protected health information (“PHI”) (as defined by the Health Insurance Portability and Accountability Act (“HIPAA”) and the Health Information Technology for Economic and Clinical Health Act (the “HITECH Act”), as amended from time to time, and patient health data as defined under any other applicable federal and/or state patient health data privacy laws and regulations (including those laws and regulations governing the privacy of mental health and substance abuse patient records).
